Translation and psychometric evaluation of the Persian version of the EORTC QLQ-ELD14 for assessing health-related quality of life in older Iranian patients with cancer.

Rapid ageing of Iran's population and the high proportion of cancers among older adults have increased the need to assess health‑related quality of life (HRQOL) in this group. Although several cancer QOL instruments exist, no validated Persian tool specifically for older cancer patients is available. This study aimed to produce a standard Persian translation and to evaluate the psychometric properties of the Persian EORTC QLQ‑ELD14. This psychometric study enrolled 319 older patients with cancer attending chemotherapy centers. Translation followed the EORTC standard forward-backward procedure. Face and content validity were examined by 10 experts using CVR and CVI. Construct validity was assessed by exploratory and confirmatory factor analyses. Reliability was evaluated by internal consistency and test-retest stability in a subsample of 30 patients. The Persian EORTC QLQ‑ELD14 achieved acceptable face and content validity after standard translation and expert review. Exploratory factor analysis yielded four main factors-Mobility, Concerns, Maintaining Purpose, and Burden of Illness-explaining 67.8% of variance, plus a single item on Family Support. Confirmatory factor analysis supported the four‑factor model with good fit indices (CFI = 0.994; TLI = 0.991). Overall internal consistency was acceptable (Cronbach's α = 0.68) and test-retest reliability excellent (ICC = 0.91). The Mobility, Maintaining Purpose and Burden subscales discriminated significantly by patients' performance status. The Persian version of the EORTC QLQ‑ELD14 is a valid and reliable instrument for assessing health‑related quality of life in older Iranian patients with cancer and can be used in clinical and research settings.
