Construction and value research of hospital-community-family linkage care model for patients with mild cognitive impairment.

OBJECTIVE: The Evaluation of the construction and value of the hospital-community-family linkage care model for patients with mild cognitive impairment (MCI). METHODS: A retrospective analysis was conducted on 60 patients with mild cognitive impairment (MCI) from November 2017 to January 2023. Patients were divided into a control group (n = 30), which received conventional care, and an observation group (n = 30), which received the integrated hospital-community-family linkage care model. The model included comprehensive diagnosis, treatment, and rehabilitation in hospitals. RESULTS: The observation group had significantly lower adverse reaction rates (P < 0.05), higher overall satisfaction (P < 0.05), and improved activities of daily living (ADL) scores (P < 0.05) compared with the control group. Cognitive assessment scores and oxidative stress indicators also favored the observation group, indicating better outcomes with the hospital-community-family linkage care model. CONCLUSION: The hospital-community-family linkage care model is vital for patients with mild cognitive impairment (MCI).
