We read with interest the study by Catanzaro et al., which evaluated the long-term safety and effectiveness of add-on fibrate therapy in primary sclerosing cholangitis (PSC) [1]. The authors provide valuable real-world data on biochemical improvements, pruritus relief, and radiological changes over a median follow-up of 34 months. However, we would like to highlight three clinically relevant aspects that were not addressed in the original study’s limitations and that merit further investigation.
