Patients present with varying stages of disease with different prognostic implications, including steatosis alone, and the more active form metabolic dysfunction-associated steatohepatitis (MASH), where steatosis is accompanied by hepatocellular inflammation and ballooning, potentially leading to fibrosis and ultimately cirrhosis.
