. The aim of the present study was to evaluate the efficacy and complications of Genioplasty technique for chin advancement in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Patient & methods 10 patients (4-male & 6 females) with OSAHS were enrolled in the current study. The patents underwent Genioplasty under general anesthesia for management of OSAHS. Primary outcome variables were assessment of tissue healing at 2nd, 3rd, 7- and 15-days day. Assessment of Sleep quality, duration, efficiency, CBAB Frequency and Aesthetic outcomes at 1st, 3rd, 6th and 12 months after surgical procedure were done. Polysomnography, Apnea-hypopnea index (AHI)& Oxygen saturation (SaO2) were assessed. Radiographic Evaluation with Panoramic view, Cephalometry & CBCT, were done at one & 12 months postoperatively to assess; Airway Area and Skeletal movement of osteomatized chin. Results statistical analysis showed significant difference in pre and post Polysomnography, Apnea-hypopnea index (AHI)& Oxygen saturation (SaO2) with significant improvement of the airway postoperatively. Conclusion Genioplasty for genioglossus advancement seemed to reduce OSAS signs and considered as an option for the surgical treatment of patients with hypopharynx obstruction. This surgical procedure is an option in patients with OSA and mandibular retrognathia to correct airway and improve retrognathia and consequently aesthetic.
