Polypharmacy encompasses inappropriate prescribing, overprescribing, and underprescribing. Polypharmacy in older adults is quite common and contributes to adverse effects, drug interactions, hospital admissions, and functional and cognitive decline. Efforts to reduce polypharmacy include criterion-based management tools, deprescribing protocols, and the use of electronic clinical decision support tools.
