A Modified Eyebrow Lift Technique Based on New Anatomy Findings of Muscular-deep Fascial System.

BACKGROUND: Eyebrow lift surgery addresses periorbital aging in Asian women, yet achieving durable eyebrow stability and adequate projection remains challenging. Leveraging recent anatomical insights into the muscular-multilayered deep fascial system, we developed an enhanced deep-plane eyebrow lift technique and evaluated its safety and efficacy. METHODS: A retrospective review of demographic data, surgical complications, and satisfaction rates was conducted for consecutive patients who underwent this modified eyebrow lift, in which the deep fascial system served as anchoring points for suspending periorbital soft tissue. Outcomes were evaluated through surgeon- and patient-reported satisfaction scores (categorized as poor, fair, satisfactory, or totally satisfactory) and by comparing preoperative and postoperative measurements of upper eyelid and eyebrow height. RESULTS: Thirty-seven patients (aged 32-73 years) were included. Mean satisfaction scores were 3.87 ± 0.41 (surgeon) and 3.71 ± 0.60 (patient). Upper eyelid height increased significantly from 4.54±0.58 mm to 5.39±0.47 mm (p < 0.001) at medial limbus point, from 4.87±0.61 mm to 6.21±0.59 mm (p < 0.001) at mid-pupil point, from 3.62±0.73 mm to 5.68±0.75 mm (p < 0.001) at lateral canthus point. Eyebrow height also improved, though not statistically significantly. No surgical complications were observed during a mean follow-up of 12.0 months. CONCLUSION: Based on recent anatomy findings regarding muscular-multilayered deep fascial system, this study explored surgical strategies for favorable and sustained periorbital rejuvenation in Asian women, with high satisfaction and no major complications.
