Completion thyroidectomy performed with at least a three-month interval demonstrates superior parathyroid preservation compared to primary total thyroidectomy, as evidenced by significantly higher postoperative parathyroid hormone levels (51.90 ± 30.26 pg/mL vs. 30.90 ± 17.10 pg/mL, p = 0.006). Postoperative calcium supplementation requirements were significantly lower in the completion thyroidectomy cohort compared to the total thyroidectomy group (4.2% vs. 25.0%, p = 0.049). A staged surgical approach provides a clear functional advantage in preserving parathyroid health and reducing the risk of transient hypocalcemia.
