Clinicians' and patients' perspectives of a mobility technician role in the hospital to promote ambulation: A qualitative study.

BACKGROUND: Hospitalized older patients are predominantly sedentary, risking functional decline. Nurses and physical therapists are primarily responsible for mobility but have many competing demands. OBJECTIVES: To explore clinicians' and patients' perceptions of mobility technicians dedicated to support inpatient ambulation. METHODS: We conducted a qualitative framework analysis informed by Normalization Process Theory (NPT). The four NPT constructs guided the development of an initial analytic framework. Stakeholders involved in the ambulation program from one health system were eligible to participate. These included patients and clinicians (nurses, physicians, physical therapists, and mobility technicians). We conducted semistructured interviews with participants from each stakeholder group. After transcription, we developed a codebook and analytical framework, and coded data using a hybrid deductive-inductive strategy. RESULTS: We interviewed 13 patients (54% male, 69% walked at least two times per day while in the hospital) and 20 clinicians (4 nurses, 7 physical therapists, 5 mobility technicians, and 4 physicians). The following themes emerged: (1) walking is important but time is a limiting factor; (2) clinicians' knowledge can motivate ambulation; (3) communication and role clarity are fundamental to success; and (4) relationships contribute to positive outcomes. CONCLUSIONS: Patients and clinicians perceive that mobility technicians have a positive impact. Hospitals considering implementing mobility technicians should develop communication plans and methods to integrate them into the healthcare team.
