The consensus aims to standardize integrated TCM and Western medicine lifestyle management in pregnant women with PMOS, and could serve as an alternative for future international consensus.
PURPOSE: Polyendocrine metabolic ovarian syndrome (PMOS), formerly polycystic ovary syndrome, is among the most common endocrine and metabolic disorders in women of reproductive age and requires lifelong management, with lifestyle intervention as the first-line strategy. Lifestyle management during pregnancy reduces the risk of PMOS-related adverse pregnancy outcomes, and combining traditional Chinese medicine (TCM) with Western medicine offers distinct advantages; however, limited high-quality clinical evidence means systematic guidance for integrated TCM and Western medicine management is unavailable.
METHODS: Using evidence review and a modified Delphi process, a panel of 25 multidisciplinary experts developed tailored lifestyle management recommendations for pregnant women with PMOS across five domains (exercise, diet, psychological health, sleep, and environment) drawing on qualitative and quantitative research, clinical guidelines, existing expert consensus statements, and nationwide expert opinion.
RESULTS: After 2 rounds of modified Delphi consultations, a final framework of 5 primary, 15 secondary, and 22 tertiary indicators was established by 25 multidisciplinary experts. With calculated expert judgment basis, familiarity with content, and expert authority coefficient values of 0.93, 0.81, and 0.87, respectively, both rounds produced a 100% response rate. The coefficients of variation dropped from 0.10-0.24 to 0.00-0.11, the mean indicator scores climbed from 3.68-4.68 to 4.16-5.00, and Kendall's W increased from 0.21 (χ2=112.49, P < 0.001) to 0.35 (χ2=184.60, P < 0.001).
CONCLUSION: The consensus aims to standardize integrated TCM and Western medicine lifestyle management in pregnant women with PMOS, and could serve as an alternative for future international consensus.
