A 43-year-old hypertensive male presented with severe abdominal pain, nausea, and constipation due to Protein C deficiency with lupus anticoagulant. CT angiography revealed thrombotic occlusion of mesenteric arteries with collateral circulation. Conservative management with dual anticoagulation and parenteral nutrition achieved clinical improvement without surgery, highlighting thrombophilia's role in mesenteric ischemia.
