Alcoholic ketoacidosis (AKA) is a metabolic disturbance triggered by excessive alcohol intake and fasting, typically presenting as a high-anion-gap metabolic acidosis with ketosis. We report a severe case of AKA with extreme hyperlactatemia, in which the patient developed shock and disseminated intravascular coagulation, emphasizing early recognition and timely intervention.
