In endemic regions, markedly elevated Brucella serologic titers with a dynamic decline after treatment support true infection even in the presence of monoclonal gammopathy. Concurrent infectious and malignant processes should be considered in patients presenting with severe renal failure and overlapping systemic symptoms to avoid diagnostic delay and optimize patient management.
