OBJECTIVES: To determine the prevalence of self-perceived burden (SPB), identify factors associated with SPB, and examine the associations of SPB with anxiety, depression, and quality of life (QOL) among patients with bladder cancer undergoing ileal conduit urinary diversion. METHODS: This cross-sectional study included 121 patients recruited from the Department of Urology at a tertiary hospital in China between January 2024 and June 2025. SPB was assessed as the primary outcome using the Self-Perceived Burden Scale. Anxiety, depression, and QOL were assessed using the Hospital Anxiety and Depression Scale and the WHOQOL-BREF, respectively. Correlation analyses were performed to examine the relationships between SPB and psychological symptoms and QOL, and multiple linear regression was used to identify factors independently associated with SPB. RESULTS: Overall, 66.94% of participants experienced SPB, including 36.36% with mild SPB, 27.27% with moderate SPB, and 3.31% with severe SPB. The impaired physical mobility (t = 3.266, p = 0.003), self-care ability (F = 4.890, p = 0.009), monthly household income (F = 2.825, p = 0.028), and caregiver health status (F = 3.791, p = 0.025) were significantly associated with SPB. SPB was positively correlated with anxiety (r = 0.410, p < 0.01) and depression (r = 0.426, p < 0.01) but negatively correlated with all QOL domains (physiological, psychological, environmental and social relationships) (p < 0.01). In the final multivariable model, anxiety, depression and monthly household income were independently associated with SPB scores, collectively explaining 28.6% of the variance. CONCLUSIONS: SPB was common among patients with bladder cancer undergoing ileal conduit urinary diversion. Anxiety, depression, and lower monthly household income were independently associated with higher SPB, while SPB was negatively associated with QOL. These findings support the potential value of psychological assessment and targeted supportive care, although longitudinal and interventional studies are needed to determine causal relationships.
