Chronic diabetic wounds pose a formidable clinical challenge owing to persistent infection and a dysregulated inflammatory microenvironment. This study reports an intelligent dual-light-responsive CCTi@CO hydrogel designed for a "Sense-Treat-Verify" closed-loop management system. By integrating pH-sensitive carbon dots and near-infrared (NIR)-active C@M nanozymes into a biocompatible matrix, the platform enables non-invasive, real-time monitoring of wound acidification under 365 nm light. Upon NIR irradiation, the system provides on-demand therapeutic intervention through robust antibacterial activity, reactive oxygen species (ROS) scavenging, and the induction of M2 macrophage polarization. Molecular assessments indicate that the hydrogel promotes accelerated angiogenesis and tissue remodeling by upregulating VEGF expression while suppressing pro-inflammatory cytokines. In vivo experiments in diabetic mice demonstrate significantly accelerated wound closure. This multifunctional platform effectively bridges the gap between diagnostic sensing and therapeutic intervention, offering a precise and responsive strategy for the comprehensive management of chronic diabetic wounds.
